Dear Parents or Guardians:
I want to take this opportunity to introduce myself and let you know about the
athletic training services that are provided to Union County Schools. I am a
Certified Athletic Trainer licensed in the states of Indiana and Ohio. An
Athletic Trainer is a sports medicine professional who is licensed in the
prevention, evaluation, rehabilitation, and education of athletic injuries. The
following is an outline of the services available to your child(ren). It
is
modeled after most collegiate sports medicine facilities, allowing better care
for your young athlete.
• FREE full service athletic
training room for minor/major injury care and evaluation, game preparation, and access throughout the entire
school year.
• Seven days a week
telephone accessibility regarding injuries and questions.
• Physical therapy provided
by McCullough-Hyde Physical Therapy and Rehabilitation Services. This allows
myself, along with my co-workers, to provide continual care for your child after
an injury.
• Educational experience: Students are encouraged to volunteer their time in the athletic training room if they have collegiate interest in the field of athletic training, physical therapy, or occupational therapy.
I openly encourage any parents or guardians to contact me or stop by the Union
County Athletic Training Room should any questions or concerns arise.

For the safety and well being of your
child, Union County High School and McCullough-Hyde Memorial Hospital have
teamed up to provide a comprehensive athletic training program. In the event of
an injury or illness associated with athletic participation, the athletic trainer
will provide injury assessment and treatment choices, as well as guidance through
the injury process. The athletic trainer may also facilitate referrals to a
specialist, including physician and/or physical therapist, if needed. While all
attempts will be made to communicate directly to the parent/guardian, the exchange of
sensitive medical information pursuant to their evaluation and assessment may be
communicated to your son/daughter directly. In that event, attempts will be made
to communicate to the parent/guardian at the athletic trainers’ earliest
convenience. Special tests, including MRI or X-rays, may be required depending on
the severity of the injury. Under no circumstances will the athletic trainer be
able to communicate the outcome of those tests prior to your son/daughter's
meeting with the physician.
All inquires regarding the test's outcome must be directed to the referring
physician. In the event the athletic trainer, working in conjunction with the
physician, receives the reports, that information will only be communicated to
the parent/guardian at their discretion. Your son/daughter’s safety is our
utmost concern. Your patience and consideration is greatly appreciated.
